Letter of Authorization
Subject: Letter of Authorization to Use Broadcom Regulatory Approvals
To: Customer’s Name (“Customer”)
Subject to CUSTOMER’s agreement with the terms of this letter, Broadcom Corporation (“Broadcom”) hereby
authorizes CUSTOMER to use the Broadcom regulatory approvals described in Exhibit 1 and the test reports to the
extent permitted by law. CUSTOMER’s authorization to use such approvals and test reports are expressly
conditioned upon CUSTOMER’s compliance with all applicable laws and any conditions set forth in Exhibit 2, and
may be revoked by Broadcom at any time in the event of any violation by CUSTOMER of such conditions or laws.
CUSTOMER may not modify the configuration of the product in any manner that would change critical regulatory
parameters or in any way that would violate the approved certification. CUSTOMER may not distribute any
tools/applications that could modify the configuration of the product in any manner that would change critical
regulatory parameters or in any way that would violate the approved certification. CUSTOMER must provide
adequate means of software version verification to ensure that no un-authorized software updates can be used by the
end user. CUSTOMER agrees to indemnify, defend and hold harmless Broadcom and its affiliates from and against
any liabilities, actions, losses, judgments, payments made in settlement, costs, suits, proceedings, demands or claims
from third parties incurred by Broadcom or any affiliate of Broadcom resulting from CUSTOMER’s failure to
comply with any conditions set forth in Exhibit 2 or any applicable laws.
This Letter of Authorization is valid only for the following Broadcom product in countries listed in Exhibit 1.

This Letter of Authorization shall be governed by the laws of the State of California, without reference to its conflict
of law principles, and is subject to the non-exclusive jurisdiction of the courts located therein. This authorization may
be extended or withdrawn at any time, at the discretion of Broadcom Corporation.
Please indicate your agreement with the terms of this Letter of Authorization by signing below and returning a copy
to me.
