Introduction
The User Guide for 2014 continues to be abridged to include only the most
important information needed for data analysis. Please see the 2006 Fetal Death User
Guide and the Guide to Completing the Facility Worksheets for the Certificate of Live
Birth and the Report of Fetal Death (2003 Revision) for more detailed information,
including detailed Technical Notes on the fetal death data.1,2
Changes to the National Fetal Death File for 2014
The 2014 fetal death file has been redesigned with substantial changes from
previous years3 in both content and format. These changes are summarized below:
Data items specific to the 2003 revision of the U.S. Standard Report of Fetal
Death4 (revised items) that were not included on the data files for 2007-2013 are
included in the 2014 file (see table A).
In an effort to improve data quality, a total of 39 data items have been dropped
from the 2014 data file. These items were dropped because they were deemed to
be difficult to collect, of poor data quality, not strongly associated with fetal
death, collected elsewhere on the Report of Fetal Death, and/or dropped from the
Standard Certificate of Live Birth5 (see table B).
The file layout has been redesigned to accommodate these changes (see 2014 fetal
death file layout)
Beginning with the 2014 data year, NCHS transitioned to a new standard for
estimating the gestational age at delivery for both births and fetal deaths. The new
measure the obstetric estimate of gestation at delivery (OE)-replaces the
measure based on the date of the last normal menses (LMP). Accordingly, the OE
2
is now used for all reporting purposes for fetal deaths and the tabulation flag
(position 10), which previously identified records with reported gestational ages
of 20 weeks or greater based on the LMP, is now based on the OE (see table 1).
Cause of fetal death data (for a subset of revised states with acceptable levels of
data quality) are being released for first time for the 2014 data year. These data
are scheduled for release in September/October of 2016. Please see:

U.S. fetal death data
United States fetal death data represent fetal deaths registered in the 50 states, the
District of Columbia, and New York City. The majority of states require reporting fetal
deaths of 20 weeks of gestation or more, or 350 grams delivery weight (roughly
equivalent to 20 weeks), or some combination of the two (see table C). The Centers for
Disease Control and Prevention’s National Center for Health Statistics (NCHS) receives
these data as electronic files, prepared from individual records processed by each
registration area, through the Vital Statistics Cooperative Program.
Fetal death data for the U.S. are limited to events occurring within the United
States to U.S. residents and nonresidents. Fetal deaths to nonresidents of the United
States are excluded from all tabulations by place of residence. Fetal deaths occurring to
U.S. citizens outside of the United States are not included in this file. Most of the
Documentation Tables included in this User’s Guide are restricted to fetal deaths
occurring at 20 weeks of gestation or more.
Availability of geographic detail

Beginning with the 2005 data year, the U.S. micro-data fetal death file no longer
includes geographic detail (e.g., mother’s state of residence). Tabulations of fetal death
data by geographic items will be available soon using the WONDER web application
available at: http://wonder.cdc.gov/. Certain geographic level data may also be available
upon request: See “NCHS Data Release and Access Policy for Micro-data and

The 2014 territories file includes data on fetal deaths occurring in American
Samoa, Guam, the Northern Marianas, and Puerto Rico (the Virgin Islands did not report
data for 2014). It includes limited geographical data such as information identifying
individual territories and counties (or their equivalent) with populations of 100,000 or
more by place of occurrence and residence.

The 1989 and 2003 Revisions of the U.S. Report of Fetal Death
This data file includes data based on both the 1989 revision of the U.S. Standard
Report of Fetal Death (unrevised) and the 2003 revision of the U.S. Standard Report of
Fetal Death (revised). The 2003 revision is described in detail elsewhere. (See the 2003

Forty-one states, the District of Columbia, New York City, Guam, and the Northern
Marianas implemented the revised fetal death report as of January 1, 2014 (see table D).
The 41 revised states are: Alabama, Arizona, Arkansas, California, Delaware, Florida,
Georgia, Hawaii, Idaho, Illinois, Indiana, Iowa, Kansas, Kentucky, Louisiana, Maine,
Maryland, Michigan, Minnesota, Mississippi, Missouri, Montana, Nebraska, Nevada,
New Hampshire, New Mexico, North Carolina, North Dakota, Ohio, Oklahoma, Oregon,
Pennsylvania, South Carolina, South Dakota, Tennessee, Texas, Utah, Vermont,
Washington, Wisconsin, and Wyoming. Together, the revised reporting areas represent
87.4 percent of all U.S. fetal deaths of 20 weeks and over. Additionally, Alaska and
Virginia implemented the revised fetal death report in 2014, but after January 1. Revised
data are combined with unrevised data from the remaining areas to create US data.
Incomplete national reporting: The use of reporting flags
The 2014 national fetal death file includes items that are not reported by all vital
statistics jurisdictions (e.g., items specific to the 2003 revision of the US Standard Report
of Fetal Death). Reporting flags were developed to help the user more readily identify
the correct number of fetal deaths for items with less than full national reporting.
Reporting flags are available for many items in the file, and positions for reporting flags
are noted along with each data item in the file layout.
Translating “blanks”
In the 2014 fetal death file, for data items which are not reported by all states,
fetal deaths to residents of a revised state occurring in an unrevised state, and fetal deaths
to residents in an unrevised state occurring in a revised state, are represented by “blanks.”

Blanks should be treated as “unknowns” for tabulations. In sum, the correct use of
reporting flags and translation of blanks will result in an accurate tally of fetal deaths for
items with incomplete national reporting. For an example of SAS code that may be used
to incorporate the correct use of reporting flags and the translation of blanks, see below.
Example of SAS code using reporting flags (and translating blanks)
The example below is for the item “mother’s education,” which is reported by the
revised reporting area (41 states, New York City and the District of Columbia in 2014).

In this example,oe_tabflg” is used to select fetal deaths of 20 weeks gestation or more.
Also, “restatus” is used to exclude fetal deaths to foreign residents (this is standard
practice for all NCHS tabulations). As well in this example, blanks are represented by
numeric values SAS code = (.). However, for some items in the file, e.g., pregnancy risk
factors, blanks are represented by character values for which the SAS code is empty
quotes (‘ ’).
Changes in reporting requirements for 2014
Arkansas - As of January 1, 2014, Arkansas amended its fetal death reporting
requirements. Arkansas now reports fetal deaths with a weight of 350 grams or more, or
if weight is unknown, when the gestational age is 20 completed weeks or more. Prior to
January 1, 2014, Arkansas state law required the reporting of fetal deaths of all periods of
gestation, although only data for fetal deaths of 20 weeks of gestation or more were
provided to NCHS.
New Mexico - As of January 1, 2014, New Mexico amended its fetal death
reporting requirements. New Mexico now reports fetal deaths of 20 weeks of gestation
or more, or if gestational age is unknown, when the fetus weighs 350 grams or more.
Prior to January 1, 2014, New Mexico only reported fetal deaths with birthweights of 500
grams or more (roughly equivalent to 22 weeks of gestation).

Changes to gestational age measurement
Beginning with the 2014 data year, NCHS transitioned to the use of the obstetric
estimate of gestation at delivery (OE) as the standard measure for estimating gestational
age.6 The OE replaces the measure based on the date of the last normal menses (LMP).6
This transition was made because of the increasing evidence of the greater validity of the
OE compared with the LMP-based measure6,7. Accordingly, beginning with the 2014
data year, gestational age data (including the tabulation flag) presented in this User Guide
and in future reports are based on the OE. Gestational age estimates differ somewhat
between the OE- and LMP-based measures and LMP-based data are presented for
comparison purposes in Table E and Documentation table 1. Note that national data
based on the OE are available only from data year 2007 forward.
Impact of change on gestational age measurement
Tabulation flag- To maximize the comparability of data by year and by state,
most of the tables on fetal deaths published by NCHS are based on fetal deaths occurring
at gestations of 20 weeks or more. These tabulations also include fetal deaths for which
gestation is not stated for those states requiring reporting at 20 weeks of gestation or
more only. Fetal deaths of not-stated gestation are excluded for states requiring reporting
of all products of conception, except for those with a stated birthweight of 350 grams or
more1. In 2014, this rule was applied to the following states: Colorado, Georgia, Hawaii,
New York (including New York City), Pennsylvania, Rhode Island, Virginia (see Table
C). The tabulation flag is used to generate the total number of records with a stated or
presumed period of gestation of 20 weeks or more based on these rules described above.
For 2014, use of the OE-based tabulation flag resulted in 23,980 records with a

stated or presumed period of gestation of 20 weeks or more. This compares with 23,999
records with a stated or presumed period of gestation of 20 weeks or more based on use
of the LMP-based tabulation flag (Documentation table 1 and Table E). In sum, use of
the different gestational age measures resulted in only a small difference (19) in the
number of total fetal deaths at 20 and greater weeks for 2014.
Total, early, and late fetal mortality- the OE compared with the LMP - For 2014,
the OE-based total U.S. fetal mortality rate was 5.98 fetal deaths at 20 weeks of gestation
or more per 1,000 live births and fetal deaths, the same as the LMP-based rate
(Documentation table 1). The OE-based early fetal mortality rate (3.16) was higher than
the LMP-based rate (2.99). Conversely, the OE-based late fetal mortality rate (2.83) was
lower than the LMP-based rate (3.01) (Documentation table 1). For the numbers and

LMP see Table E.
