IMPORTANT INFORMATION ABOUT THE UC STUDENT HEALTH INSURANCE PLAN (UC SHIP)!
Attention students (and parents)! This is a courtesy email reminder on behalf of the Student Health Insurance
Office at the Ashe Center. We understand that you’re busy with finals quickly approaching, but please take a few
minutes to read the following information regarding healthcare coverage for 2014-2015. You will find updates on
the new waiver criteria to waive UC SHIP moving forward, important deadlines and reminders for waiving, utilizing
UC SHIP, purchasing BruinCare and other health-related reminders.
WAIVING FOR WINTER AND SPRING 2015
This Fall, the minimum coverage criteria required to waive enrollment in SHIP presented new challenges for
students, parents and UC administrators alike. In response, a streamlined, less restrictive criteria was voted upon
and will be effective Winter 2015.
If you have successfully waived for Fall 2014, no further action is required for the P/Y 2014-15. If your waiver was
not successful, the Fall 2014 denials will stand and there will be no re-evaluation or refunds of Fall 2014 waiver
denials or denied appeals based on the new criteria. However, from December 1 through December 20,2014 you
may be able to waive enrollment in SHIP for Winter/Spring Quarter 2015 or Spring 2015 Semester (for Law and
Med students) if your policy meets the new less restrictive criteria.
Students who forgot to waive for Fall 2014 will have the opportunity to waive for the coming Winter/Spring 2015
Qtr. or Spring Semester (for Law and Med School) from December 1 through December 20, 2014. You will find the
new waiver criteria on the Ashe website under the Insurance tab here.
THE NEW WAIVER CRITERIA FOR ACADEMIC YEAR 2015-2016
The restructured criteria (also include hyperlink here) will be the Waiver Criteria for Academic Year 2015-16.
Students and their families can now begin to plan for the next academic year.
Covered California open enrollment begins November 15, 2014. Please review your plan options carefully by
considering cost, benefits and the network of preferred providers as compared to your campus SHIP program.
FAQs: WAIVERS & ENROLLMENT
I forgot to waive for Fall. How can I waive for the rest of the academic year?
The waiver application period will be open from December 1 through December 20, 2014 for all quarter and
semester students. Waiving UC SHIP is an online process completed through the Student Health Insurance website.
Transactions are always saved for a processed waiver and you will be notified automatically if your waiver was
approved or denied. If approved, a confirmation letter will generate and be sent to your email inbox. Be mindful
that it does not go to spam or junk mail. If denied, you have 14 business days from the date of denial to appeal by
submitting the appeal form and requested documents.
I waived UC SHIP, but can I be treated at The Ashe Center?
As a registered student, you are still eligible to receive services at The Ashe Center according to nominal fees, e.g.
A basic office visit is $12. All charges incurred at the Ashe Center are posted directly to your BruinBill account.
I waived UC SHIP for the fall, but can I cancel my waiver to enroll for the next academic term?

The waiver cancellation period is from December 1 until December 20, 2014 for all quarter and semester students.
In order to do so, return to the same online waiver application to select the option to cancel and subsequently
delete your waiver. Once canceled, the UC SHIP premium fee will repost to your BruinBill within 3 business days for
due payment. Please visit the Student Health Insurance site for coverage dates that uniquely apply to you.
UC SHIP & THE AFFORDABLE CARE ACT
UC SHIP is an excellent healthcare program for UCLA registered students. For the price of a bronze plan, you
receive coverage at the platinum plan level with access to reputable Anthem network providers and specialists in
the UCLA Health System. If you or your parents are interested in how UC SHIP differs from the health exchange
plans, please download and print this informational sheet to compare your options.
Please note that if you plan to waive UC SHIP between December 1 and December 20, 2014 for winter and spring
terms, you must be enrolled in a health exchange plan by December 15, 2014 in order for your plan to be in effect
by January 1, 2015. The effective date must parallel with UC SHIP’s start date. Additionally, your plan must also
comply with the UC SHIP Waiver Criteria for 2014-15.
UC SHIP includes medical, vision, dental and UCLA Counseling and Psychological Services (CAPS) and Behavioral
Health Services coverage. The undergraduate premium fee is $646.10/quarter and the graduate UC Student Health
Insurance Plan or G-UC SHIP fee premium is $1,003.70/quarter or $1,505.55/semester (Law and Med students).
UC-SHIP DURING THE HOLIDAYS
The Ashe Center will be closed from Wednesday, December 24 through Friday, January 2, 2015. We will reopen
on Monday, January 5, 2015.
Traveling abroad: Anthem Blue Cross is your insurance carrier. Medical assistance outside of the U.S. is provided
through the Blue Card Worldwide Program. Providers are available through the Service Center at +1 (800) 810-
2583 or collect at +1 (804) 673-1177 if you are unable to find a suitable provider. You can also go online to
www.bluecardworldwide.com and search within a 30-mile radius of the nearest city for a list of doctors, hospitals,
pharmacies, etc. We strongly encourage you to make your own preferred list in case of emergencies. Unlimited
benefits are furnished for medical evacuation and repatriation if needed.
Away from Campus: If you will be away from campus but still within the U.S., medical assistance is available in
many ways:
Secure-message your Ashe primary care provider through the Patient Portal for a referral to an Anthem
network provider.
If you cannot reach your Ashe primary care provider, contact the Student Health Insurance Office during
regular hours at (310) 795-5614 or asheinsur@ashe.ucla.edu. Please include your UCLA SID.
If the Ashe Center is closed, you may utilize our 24/7 Nurse Line for confidential medical advice at (877)
351-3457 to determine if it is an urgent situation or otherwise.
You are encouraged to seek emergency care through an emergency room or urgent care clinic by finding
one closest to your area at www.anthem.com. Any follow-up appointment that is scheduled must be
communicated to the Student Health Insurance Office as a referral may be needed for further treatment
or diagnosis.
NON-REGISTERED/VOLUNTARY SHIP

Are others “depending” on you? Options are available for dependents of registered and insured UC SHIP students
to purchase a Non-Registered Voluntary UC SHIP policy. Contact Wells Fargo Insurance Services, a contracted
affiliate with UC SHIP and its insurance carrier, Anthem Blue Cross, at (800) 853-5899 for the enrollment
application, fee payments, and benefits details. The process does not automatically renew, so please re-enroll your
dependents every academic term. More information on dependent enrollment can be found through the Student
Health website here.
Need to extend UC SHIP enrollment after you’ve left UCLA? Registered UC SHIP students who wish to continue
their coverage after graduation (graduates and undergraduates) or the end of term (graduates only) may be
eligible to purchase the Non-Registered Voluntary UC SHIP policy strictly for one additional term. You must have
been insured in the previous academic term to qualify. You have 30 calendar days prior or after your end date of
coverage to begin enrollment. Contact Wells Fargo Insurance Services at (800) 853-5899 for the application and
direct payment. Your coverage will maintain the same benefits, co-payments, co-insurance and Ashe referral
requirements until the end of the term in which you are enrolled.
GENERAL HEALTH & ASHE CENTER REMINDERS
Hepa-what? Students who are under the age of 19 are required to provide proof of Hepatitis B immunization prior
to enrollment. For clearance, an online questionnaire must be completed through the Student Health Website
Hepatitis B site.
USE CAPS! UCLA Counseling and Psychological Services (CAPS) are available to all registered students. Remember
that all registered students insured by UC SHIP must initiate behavioral health assessment/care through CAPS first.
Referrals are sent to UCLA Neuropsychiatric Behavioral Health Services. Visit CAPS’ website for more information
