Rebate Terms and Conditions:
This form must be fully completed with purchaser information
(Fields 1-7) filled out with valid and current data. Offer valid only in
the US and Puerto Rico. Rebate paid in U.S. currency in the form
of a check. All rebate requests must be postmarked by February
15, 2007. Any requests postmarked or received after applicable
date will be ineligible. Receipt must include a date that falls within
the mail-in rebate offer period. No reproductions of the UPC codes
will be accepted. Note: Submissions with serial number S/N code
instead of the original UPC code will be considered invalid.
Creative Labs is not responsible for late, lost, misdirected or
postage due mail. This offer is only available to end-users. No

honored. If terms and conditions are not met in Creative’s sole
discretion, rebate will not be sent. All decisions by Creative are
final. Rebate checks are void if not cashed within 90 days of
issuance and cannot be reissued. Offer limited to two rebate
