To receive your $20 usd Visa® Prepaid Card rebate from Symantec, please follow these six steps: 1. Purchase Norton 360™ Version 5.0 between 2/16/11 and 2/24/11 at Fry’s Electronics or Frys.com. 2. Read and complete this rebate request form. 3. Enclose the original UPC code from Norton 360 Version 5.0. The UPC code is a 12-digit bar code found on the bottom of box. If you purchased this product in a DVD case, the UPC code is located on the back of the case. Photocopies will not be accepted. 4. Enclose a legible copy of your dated sales receipt from Fry’s Electronics or Frys.com with the purchase price circled of Norton 360 Version 5.0. Handwritten or altered documents will not be accepted. If you purchased this product online, your dated sales receipt is your confirmation email from your online purchase. 5. Make a copy of your rebate submission for future reference. Originals submitted become Symantec property and will not be returned. 6. Mail your submission to the following address (must be postmarked within 30 days of purchase): Symantec # 11-44854 P.O.
