INTRODUCTION
This publication is intended to help you understand and interpret entries on the driving records
available from Oregon Driver and Motor Vehicle Services (DMV).
RECORD TYPES
There are five types of driving records available: Certified court prints; nonemployment driving
records; nonemployment, open-ended driving records; and employment driving records.
The certified court print driving record includes most traffic violations and accident
entries for at least the previous five years. Serious criminal traffic offenses, such as
driving under the influence of intoxicants, may remain on the record permanently.
The nonemployment driving record has entries for at least the previous three years.
This record lists all nonemployment-related accidents, convictions, suspensions,
cancellations, revocations, and diversion agreements.
The nonemployment, open-ended driving record contains the same information as a
nonemployment driving record but is not limited to three years and is available only for
the purpose of providing insurance discount to an individual.
The employment driving record is a three-year record that is limited to convictions and
accidents the driver had while on the job.
The certified court print with CDL medical certification driving record is a certified
court print that includes commercial driver license medical certification information.
For each record type there are two versions of the record available: (1) a version containing
the driver’s personal information and (2) a version that has the driver’s personal information
removed (sanitized). Personal information is removed from a record if a requestor does not
qualify to receive it under Oregon’s Record Privacy Law (ORS 802.175-802.191) or does not
have a written, notarized permission slip from the individual whose information is requested.
Personal information is: driver license, driver permit or identification number; name; address;
and telephone number.
Under Oregon’s Record Privacy Law, only certain qualified entities are allowed to receive
personal information. These include such entities as attorneys, legitimate businesses,
insurance companies, police agencies, and other government agencies. See the Records,
Privacy & Account Holders section on the DMV website (www.oregondmv.com) for additional
information.
FIXED ENTRIES
Fixed entries generally include information we record for each driver/customer, such as name,
address, and date of birth. This portion of the record also contains information about any
identification card or Oregon driving privilege the person holds, as well as endorsements,
restrictions, and expiration dates.
VARIABLE ENTRIES
Variable entries are those that make up an individual’s driving history, such as accidents,
convictions, suspensions, revocations, cancellations, and other administrative actions. Variable
entries may also include information about the type(s) of license, permit, and/or endorsement
the person holds (or has held in the past).

CERTIFIED COURT PRINT DRIVING RECORD
This is a certified court print that includes convictions for traffic crimes, commercial driver license entries,
diversion agreements, and alcohol rehabilitation entries; convictions for minor offenses and accidents will show
for at least five years. Suspensions, cancellations, and revocations also are included, along with miscellaneous
administrative entries and multiple status entries. It may also contain some out-of-state commercial driver entries.

This is a certified three-year record that includes Oregon accidents, diversion agreements, and convictions other
than those in the employment driving record. The record may contain some commercial driver license entries,
some out-of-state commercial entries, miscellaneous administrative entries, and multiple status entries.

NONEMPLOYMENT OPEN-ENDED DRIVING RECORD
This record is available only to an insurer, insurance support organization, and the person to whom the record
pertains for the purpose of providing a discount to the individual. It contains the same information as a non-
employment driving record but is not limited to three years.

(b) Account number used by DMV, or the account number charged.
(c) Page number of the record.
(d) Customer number (Oregon driver license, driver permit, identification card number or reference
number assigned by DMV to create a driving record).
(e) The label “NAME” appears followed by the driver’s name.
(f) The label “ADD” or “WRK” appears on this line. If “ADD” is displayed, the address shown is the
person’s residence address. If “WRK” is displayed, the address shown is the person’s
employment address.
(g) City, state, zip code (nine-digit number).
(h) Date of birth (zeros if unknown).

(j) Expiration date of driver’s license. 00/00/00 indicates no license has been issued or expiration
date has been voided because of another reason such as a revocation or cancellation.
(k) License type. Indicates the class of license or “I” if a person has been issued an ID card or
“OUTS” if person held an out-of-state license when record was created, i.e. Oregon record was
created to record a conviction. Refer to License Classes for explanation of license types and
Instruction Permit Classes for permit type.
(l) The Policy or ID field is available for use identification, such as name or case number of the
person ordering the record. A maximum of 20 spaces may be used.
(m) Endorsements attached to a license are displayed here.
CODE ENDORSEMENT
H Any vehicle which carries hazardous materials (HAZMAT) that requires placards.
M Motorcycle endorsement for any motorcycle regardless of engine displacement.
N Tank vehicle is designed to transport any liquid or gaseous material within a tank or tanks
having an individual rate capacity of more than 119 gallons and an aggregate rate
capacity of 1,000 gallons or more that is either permanently or temporarily attached to the
vehicle or the chassis. “Tank vehicle” does not include a portable tank having a rated
capacity under 1,000 gallons.
P Passenger vehicles designed to carry 16 or more persons, including the driver.
S School bus used to transport students to/from school or school events.
T Double/triple trailers.
X Combined hazardous materials/tank endorsement.
Y Farm endorsement (Class A).
Z Farm endorsement (Class B).
(n) Displays “YES” if person has commercial license or “NO” if person does not have a commercial

(p) Original date. Date the record was created.
(q) Expiration date of ID card. If a date appears in this field, it is the expiration date of the
identification card; otherwise, the word “NONE” appears here.
(r) 1st licensed. Indicates when the person was first issued an Oregon license. 00/00/00 indicates the
person’s first license was issued prior to March 1, 2000, or no license has ever been issued.

The following entries do not appear on every driver’s record. They appear on a record only if the
specific issuance type occurs.

Class A Commercial May operate any vehicle or combination of vehicles with the proper endorsements.
Class B Commercial May operate any single vehicle with proper endorsements. May tow a trailer up to
10,000 pounds GVW or GVWR, whichever is greater. May tow a vehicle more than
10,000 pounds GVW or GVWR, whichever is greater, as long as the combined
vehicle weight does not exceed 26,000 pounds GCVW or GCVWR, whichever is
greater.
Class C Commercial May operate any single vehicle, with proper endorsements, with a GVW or GVWR,
whichever is greater, of not more than 26,000 pounds that is designed to carry more
than 16 people (including the driver) or is transporting hazardous materials in
quantities that require placarding by law. May tow a trailer up to 10,000 pounds
GVW or GVWR, whichever is greater. May tow a trailer more than 10,000 pounds
GVW or GVWR, whichever is greater, as long as the combined vehicle weight does
not exceed 26,000 pounds GCVW or GCVWR, whichever is greater.
Class C May operate any single vehicle, with proper endorsements, with a GVW or GVWR,
whichever is greater, of not more than 26,000 pounds that is designed to carry 16 or
fewer persons (including the driver) and not transporting hazardous materials in
quantities that require placarding by law. May tow another vehicle with a GVW or
GVWR, whichever is greater, of less than 10,000 pounds. May tow a trailer more
than 10,000 pounds GVW or GVWR, whichever is greater, as long as the combined
vehicle weight does not exceed 26,000 pounds GCVW or GCVWR, whichever is
greater. May operate a moped. Firefighters operating emergency fire vehicles are
also included in this class, as are operators of recreational vehicles being used for
personal use.

STATE OF RECORD
DMV uses this entry to show that a record on the Commercial Driver License Information System
(CDLIS) has been changed to show Oregon as the State of Record (SOR), because a license was
issued or driving privileges were reinstated. It is also used when another state issues a driver license to
a person who has an Oregon record on CDLIS.

RESTORE CDL ENTRY
This entry is used when a CDL is downgraded to a Class C noncommercial license or a Hazmat
endorsement is removed while waiting for the results of a security check. The downgrade could be for a
number of different reasons. The segment indicates the driver may restore a CDL if they meet eligibility
requirements within the permitted restoration date.

SECURITY CHECK SEGMENT
This segment indicates when a customer has started the security check process for a Hazmat
endorsement.

ACCIDENTS
Accidents are posted to a driver’s record after a reportable accident. Oregon law requires an accident
be reported to DMV when one or more of the following occurs:
An injury or death to any person;
Damage to the vehicle you were driving is over $2,500;
Property damage (not including a vehicle involved in the accident) is over $2,500; or
Damage to any vehicle is over $2,500 and any vehicle is towed from the scene due to damages
from the accident.

format is an example of an accident occurring in Oregon. The second format is an example of an

FINANCIAL RESPONSIBILITY
Financial responsibility means the ability to pay for damages resulting from an automobile accident.
The Financial Responsibility Law requires that proof of financial responsibility be carried in any vehicle
operated on public highways or roads. Future responsibility must be filed and maintained for three
years with DMV by persons suspended for the following reasons: uninsured accident, fail to furnish
proof of insurance, driving uninsured, and false certification.
Future responsibility filings are also required at the end of mandatory suspensions or revocations
issued by a court or DMV. These include traffic crimes such as reckless endangerment, menacing,
criminal mischief, failure to perform the duties of a driver in an accident, reckless driving, DUII, etc.

effect indefinitely. Although a driver is eligible to reinstate the revocation after five years, driving
privileges remain revoked until the driver complies with all reinstatement requirements.

IGNITION INTERLOCK DEVICE
A person is required to have an Ignition interlock device (IID) on any vehicle they operate. The initial
length of the requirement for an IID depends on the driver record and can be for one year, two years or
five years.
The IID requirement starts from the date ordered by the court for a DUII diversion agreement; the end
dates of the suspension or revocations caused by the conviction; and during a DUII suspension if the
driver applies for a hardship permit.
At the end of the initial IID requirement, the person must submit proof of no IID violations for the last 90
days of the initial requirement or DMV will impose an indefinite IID requirement until the person can
submit the proof of no IID violations.

SUSPENSION, REVOCATION, AND CANCELLATION
Suspension is the temporary withdrawal of driving privileges. Revocation is the termination of driving
privileges. Cancellation means the driver’s license or ID card is void and terminated. All these actions
affect the status of Oregon driving privileges.
NOTE: Withdrawal (WTHD) is an entry used to record an action taken in another state. DMV records
“Withdrawals” for historical/informational purposes. Withdrawal entries should not be used to determine
if a driver has valid driving privileges in Oregon or in another state.
The following are three examples of the format used for Suspensions, Revocations or Cancellation
actions taken by Oregon. Following the Oregon examples is an example of a Withdrawal segment
using the ACD format to record actions taken in other states; it is the standardized format used to
transfer record information from state to state.

EXTENSION CODES
When DMV imposes suspensions for failure to appear on a traffic crime, the law requires DMV to note
the type of crime. DMV notes the type of crime using extension codes. The extension code and offense
literal displays directly below the action to which it corresponds (see field (j) in Suspension,
Cancellation, Revocation, page 23). See the Standard Convictions Abbreviation Table starting on
page 45 for a summary of the offense literal.

COLUMN 1 – LITERAL
This column contains either the Oregon conviction abbreviation (literal) or the out-of-state ACD code.
Oregon conviction abbreviations are three to 12 character alpha/numeric abbreviations for the offenses.
ACD codes are the standardized codes as determined by the AAMVA Code Dictionary (ACD) used to
transfer driving record information from state to state. ACD codes are three characters; an alpha
character followed by a two-digit number. These codes appear on Oregon driver records as convictions
or withdrawals:
When an out-of-state commercial licensed driver moves to Oregon and the driver’s previous
state driving record is transferred electronically to Oregon; or
