Forms Needed for Students/Parents FIRST STIMS (Student Team Information Management System) - All students on the Team must apply to be a member of Team 1510. Follow the Instructions for the Electronic Team Roster and Release Form carefully. These REQUIRE parent’s email account for approval; NO release form or NOT on Electronic Team Roster = NO PARTICIPATION (Parents attending the January Kick Off event MUST also have a paper copy of the Consent & Release form available on the FIRST website filled out to hand to me at the event.) BSD website Volunteer Background Check - REQUIRED of ALL Parents and Mentors who are around the students for more than one minute w/o supervision. https://www.beaverton.k12.or.us/depts/CCI/volunteer-engage/Pages/default.aspx - Follow instructions for Parent Volunteer for Westview HS STEM education. - Click on the Volunteer Background Check (black background/white hand). - email Coach Hendel when you have completed the form (hendelf@frontier.com). When I check with BSD you can’t participate if you are not on the approved list. BPS Robotics Membership Contract/Code of Conduct – This document states expectations and is available online at website http://www.bpsrobotics.org/ click on Team 1510 then select Team Handbook on the right, or use this direct link http://www.bpsrobotics.org/1510/index.html Parent and Student read Team Handbook and sign Code of Conduct form. BSD RL+HH – Release of Liability and Hold Harmless for the school district for non-school sponsored trips and competitions. Enter “Robotics”, “Robotics” and “BSD” in the three spaces. Form B – BSD Transportation & Release from Liability for the school district. Activity is “Robotics” and times/dates “Various” Form C – BSD Private Auto bottom section so your student can ride with any BSD approved driver so you don’t have to transport your student every time. Coaches and most Mentors are approved drivers. If you wish to be an approved driver you will have to fill out a separate Form C on the top and back and supply a copy of your driver’s license and current insurance coverage card for each vehicle you will use. Scan and send a PDF file to me, or I can scan those and make copies before submitting to Westview. Form E – BSD Student Contract & Release agreement. Form F – BSD Health History so emergency care can be administered to your student when we are traveling. Requires a copy of your health insurance card and the student’s ID card. No insurance = no participation. I can scan them if you wish. Otherwise please scan and send a PDF file to me. Students, Parents and Mentors who will be traveling with the team on common carriers must supply their full name on government issued photo ID card and birth date. All common carriers (airlines, trains) require this information before tickets can be purchased and released. St. Louis trip is the one this is needed. STUDENTS are required to carry a photo ID card at all times!
