In keeping with our “Going Green” mission, PALS and earth creations, a local business specializing
in natural dyed organic cotton and hemp clothing, are partnering together to help raise funds for the
Children’s Department of the Vestavia Hills Library in the Forest. When you purchase either of the
pictured organic cotton clay dyed logo shirts between Oct. 18th and Nov. 1st.,
45% of the proceeds go back to the Children’s Dept.

1) ALL ORDERS WILL BE AVAILABLE FOR PICK-UP AT THE LIBRARY IN EARLY DECMEBER.
2) IF YOU WOULD LIKE YOUR ORDER SHIPPED DIRECTLY TO YOU THERE WILL BE SHIPPING CHARGES APPLIED. PLEASE SPECIFY IN NOTES AT CHECKOUT.
3) YOU MUST USE THE code PALS at online check out to insure THAT PALS RECEIVES THEIR MONEY.
4) IF YOU WOULD LIKE TO PAY BY CHECK PLEASE MAIL THIS ORDER FORM IN WITH YOUR CHECK made out to earth creations, inc.
