Required documents for applicants eighteen and older
• Copy of a valid Louisiana Driver’s License issued a minimum of six months and ONE OF THE
FOLLOWING
:
• Copy of applicant’s current Louisiana voter’s registration card
• Copy of Louisiana vehicle registration in applicant’s name, if the applicant is married and vehicle registration is in spouse’s name, copy of marriage
license must accompany the application
• Copies of applicant’s Louisiana state resident personal income tax return (form IT-540) for the two filing periods immediately prior to the date of
application, these tax returns must be stamped “received” by the Department of Revenue
Required documents for applicants under the age of eighteen
• An original certified birth certificate from the Department of Health (Birth certificate will not be returned if applying by mail)
• Copy of both parents Louisiana driver’s license

Residency
Certification
I qualify for resident licensing having complied with ALL of the residency certification requirements as I hereby attest to the following statements:
1. I have resided in Louisiana continuously for at least the past six months.
2. I am registered to vote in Louisiana; or, I am not registered to vote anywhere.
3. I am in possession of a valid Louisiana Driver’s license; or, I am not licensed to drive anywhere.
4. I am in possession of Louisiana registrations for all motor vehicles I own within Louisiana; or, I do not own any motor vehicles within Louisiana.
5. I have filed a Louisiana State Income Tax return and complied with state income tax laws and regulations; or I did not earn an income.

will be returned.
• Lifetime licenses are valid anywhere in the state. If you buy a lifetime license and later move out of
the state, your license will remain valid any time you return to Louisiana to hunt and/or fish.
• Lifetime Licenses are issued from the Baton Rouge office only.

• Requests for a duplicate lifetime license to replace a lost, stolen, or destroyed license must be
accompanied by a completed and notarized affidavit for duplicate license.
The cost of
duplicates
is $7.50.
• A Lifetime card will be issued to all individuals who purchase a Lifetime License. In order for your
Hunter Education number to appear on your Lifetime Hunting or Lifetime Combo card, you will need to
submit a copy of your Hunter Education card at the time of applying for a Lifetime license. If proof of
Hunter Education is not submitted at the time of applying, the Lifetime card will reflect “00000000” in
the Hunter Education field. At any time in the future, if you would like a Lifetime card to reflect your
Hunter Education number, you can apply for a Duplicate Lifetime card by submitting the notarized
affidavit, proof of your Hunter Education card and the fee of $7.50 to the Louisiana Department of
Wildlife and Fisheries, Attn: Licensing, P. O. Box 98000, Baton Rouge, LA 70898-9000.
• If the application is for a gift, the application must be accompanied by all supporting documents
required, and the purchaser can sign the application.
