Course Description
The 2012 Mayo Clinic Alumni Association International CME Program, in response to
requests from previous course attendees, will address a variety of relevant topics in
medical education, research and clinical practice. The conference format balances topics of
general and medical specialty interest and physician leadership information with particular
attention to preventive screening, integrative and holistic approaches to health, aging,
administration, research breakthroughs and innovation through lecture and an interactive
speaker panel. Faculty members will be available during the breaks to answer questions
and to discuss topics with course participants.
Intended Audience
Physicians and scientists trained at Mayo Clinic from Europe, the Mediterranean, the
Middle East, the United States and all members of the Mayo Clinic Alumni Association.

The Mayo Clinic Alumni Association International Scientific CME Education Program
will be held October 22-25, 2012, in Jerusalem. Course headquarters is the Dan Jerusalem
Hotel situated on the historic slopes of Mount Scopus, overlooking the Old City of
Jerusalem and surrounding hills of Judea. http://www.danhotels.com/JerusalemHotels/

All course accommodations are Executive or Executive Old City view rooms, which include
breakfast (see registration form). The number of Executive and Executive Old City view
rooms is limited; early registration is encouraged. Participants should schedule their flights
to arrive at Ben Gurion Airport in Tel Aviv, Israel, on Monday, October 22. Tel Aviv is the
main international airport and is a 45-minute transfer to Jerusalem. October temperatures
in Jerusalem range from 66-77 F for highs to 49-57 F for lows.
All attendees must possess a valid passport. There is no visa required for Israel (or Jordan
for post-conference participants) for citizens from the USA or Canada. (Other passport
holders please verify your immigration requirements.) US passports must be valid for six-
months after returning to the US from your trip.
The International Scientific CME Education Program will begin with an evening reception
on October 22. The scientific sessions are scheduled from 7:30 a.m. 12:30 p.m. on October
23, 24, and 25. Participants may also register for a pre-conference program which includes
three additional nights and tours in and near Jerusalem. A 10-night post-conference
program is available and features: four nights in Israel that includes a Friday Shabbat
program in Jerusalem, Masada and the Dead Sea, and six nights in neighboring Jordan that
includes Petra, the Wadi Rum and Amman. The post-conference tour ends in Amman on
Monday, November 5. Additional information on these programs is contained within this
brochure.

Scientific CME Education Program Information
Meeting Headquarters: Dan Jerusalem Hotel
Monday, October 22, Opening Reception, 7:00 9:00 p.m.
The reception is included for all registrants; spouse/guest welcome with reservation (see
registration form and deadline). A variety of hot and cold hors d’oeuvres and beverages
will be provided along with cultural entertainment. Weather permitting, the reception
will be held on the beautiful outdoor terrace of the Dan Jerusalem Hotel, overlooking Old
City Jerusalem.

The gala is included for all registrants; spouse/guest welcome with reservation (see
registration form and deadline). This exclusive gala venue features a unique overview
of the historic Old City of Jerusalem and is spectacular. The setting sun and illuminated
walls will create a glowing backdrop for dinner and entertainment.

Tuesday, October 23
Afternoon and Evening Jerusalem City Tour and Dinner
Beginning at the Mount of Olives, with a panoramic view of the ancient city of Jerusalem,
you will tour the Old City and the Western Wall Tunnels accompanied by professional
English-speaking guides. Highlights feature the four neighborhoods of Jerusalem which
are named according to the majority ethnic affiliation who live there. This Old City
orientation includes a walking tour through Jaffa Gate and down David Street where the
ethnic quarters surround you, including Christian, Armenian, Jewish and Muslim. A
visit to the unique Western Wall Tunnels takes you on a journey through the fascinating
timeline of history and archaeology of the city. Dinner will be at Bulghourji, a classic
renowned Old City restaurant housed in an ancient bulgur factory. Hotel departure
at approximately 1:30 p.m.; return approximately 9:00 p.m. Business casual attire is
appropriate; good walking shoes are required for the uneven Old City walkways.

Wednesday, October 24
Afternoon and Evening Tour featuring the new Holocaust History Museum, the
Tower of David and Dinner at the American Colony. Yad Vashem, the Holocaust
History Museum, is a powerful experience for visitors to Israel and has recently been
renovated. The new museum places an emphasis on the history of the Holocaust from
a human perspective and uses multimedia to present a historical narrative interwoven
with personal stories. Following your visit, continue on to the rooftop of Notre Dame
for time to reflect and converse with your fellow alumni attendees before experiencing
the beautiful sunset sound and light presentation at the Tower of David. Dinner follows
at the American Colony Hotel in its beautiful Arabesque Restaurant. Hotel departure
at approximately 1:30 p.m.; return approximately 10:00 p.m. Business casual attire
is appropriate. Spouse/guest welcome; all participants must have reservations (see
registration form and deadline).
Friday, October 26
Friday Shabbat Program and Dinner. Extending your stay for one additional day will
allow for free time, a visit to the Western Wall and the opportunity to experience a
traditional Shabbat. Dinner is included. Spouse/guest welcome; all participants must

Cancellation Policy
Reservations for accommodations and optional programs must be made before July
31, 2012. All reservations are on a first-come basis. Cancellations after this date are
subject to penalty and are the individual’s responsibility. Hotel rates at the MCAA rate
are available two nights pre-conference and two nights post-conference. Important
notice: the MCAA rates quoted are for a 4-night minimum stay. If less than 4 nights
are booked for any individual reservation, the hotel reserves the right to amend the
quoted rate. Cancellation policy: if cancellation is required, all efforts will be made to
regain payments required for hotel banquet services and/or optional tours confirmed.
Recouping payment is based on the payments already paid by Concierge Services, LLC,
to hotels and land operators, which are contractual. Cancellation penalties may apply
and are the participant’s responsibility. Cancellation insurance is highly recommended
and will be sent on request to interested individuals.

Participant flight schedules for those not participating on any pre- or post-conference
programs should plan to arrive on Monday, October 22, 2012 in Tel Aviv, Israel (TLV).
The program ends with the finale dinner on Thursday, October 25 and departing flights
can be made for Friday, October 26. If attending the finale dinner on October 25 and you
wish to depart on October 26, select a flight departing later than 3:00 a.m.
MCAA room rate will hold for two nights pre- and two nights post-conference program
for those not extending on offered pre- or post-conference programs.

Pre- and Post-Conference Extended Tours
Pre-Conference Program: 4 days/3 nights additional touring in and around Jerusalem,
featuring a full day comprehensive touring experience in Bethlehem and a full day
comprehensive touring in Tel Aviv, and a culinary guided tour in the Old City.
Upgraded rooms at the Dan Jerusalem Hotel (same accommodations as the Scientific
CME Education Program; you will not need to change rooms). Arrival on Friday, October
19 (depart USA on Thursday, October 18) and inclusions feature transfer and baggage
handling from Tel Aviv airport and at hotel, English-speaking guides, Tour management
by Concierge Services, LLC, daily breakfast, 2 lunches and 2 dinners, and all taxes
and gratuities. Minimum participation of 20 people required to hold the price of $1,100
per-person. Should minimum numbers not be achieved, registrants will be fully refunded
deposit paid.
Post-Conference Program: 10 nights in northern and eastern Israel and Jordan.
Friday participants will enjoy a Shabbat in Jerusalem. Accommodations are available for
an additional two nights at the Dan Jerusalem Hotel. On Saturday, October 27, tour to
northern Israel with visits at Capernaum, an ancient fishing village on the shore of the
Sea of Galilee, and the Mount of Beatitudes. Lunch and a visit to Nazareth will also be
included. On Sunday, October 28, the group will depart Jerusalem for Masada National
Park and the Dead Sea. Masada is an historic and famous archaeological site in Jewish
history. Full touring will be enjoyed as will lunch and a float in the Dead Sea, the lowest
point on Earth at 380m below sea level. Changing rooms and shower facilities will be
available. Two nights will follow in upgraded accommodations at the amazing spa

The group will cross into Jordan at Eilat on the Red Sea on Tuesday, October 30. The
following six nights in Jordan will highlight the magnificent history and environment
including the Wadi Rum (overnight at deluxe King Aretas IV Luxury Camp http://
www.kingaretascampjordan.com/), two nights at spectacular Nabatean civilization
site, Petra—the rose red city carved into stone (http://www.moevenpick-hotels.com/
en/pub/hotels_resorts/worldmap/petra/welcome.cfm) in upgraded superior rooms,
and three nights at the Grandy Hyatt in Amman in upgraded Club category rooms
(http://www.hyatt.com/hyatt/wwg/hotels/ammgh/index.jsp). Fully escorted and
professionally managed, the inclusive tour will feature a luxury tented-camp experience
in the Wadi Rum made famous by T.E. Lawrence (Lawrence of Arabia); comprehensive
Petra tours; the massive Crusader fortress at Kerak; Madaba, the Mosaic City; Mount
Nebo, mountaintop memorial of Moses with captivating view over the Jordan Valley;
Jerash—a remarkably preserved Roman town north of Amman, Umm Qais, and an
Amman city tour to include the vast Souq (market). Many extras are included as well as
daily breakfast, lunch and dinner (excepting dinner on day 6). Extras include 4 x4 jeep
excursion in the Wadi Rum, horse ride in Petra from main entrance to the Siq (narrow
rock entrance to Petra), all gratuities, baggage handling and more.
Per-person for the above unique and exclusive package: $3,600.00 Minimum
participation of 20 participants is required in order to hold the price of $3,600 per-person.
Should minimum numbers not be achieved, registrants will be fully refunded deposit
paid. Passports must be valid through May 5, 2013, for those extending on this tour. If
passports expire before this date, you should renew your passport before departure.

Pre- and Post-Scientific CME Education Program
Touring Extensions Terms and Conditions
Reservations must be accompanied by a 50% per-person deposit and a completed and
signed registration form. Final payment is due by July 31, 2012. Deposit and final
payments can be paid by Visa/MasterCard/American Express only. Tour confirmations
are based on date deposits are paid and on a first-come basis. Minimum number of
20 participants required for either program to hold the pricing as outlined. Concierge
Services, LLC, will invoice you for the final payment due. Should minimum numbers not
be achieved, all deposits paid will be fully refunded.
Travelers are responsible for obtaining proper passports and/or other international
identification documents that you require and for assuring their validity. US Passports
must be valid through April 27, 2013 for those returning to the USA on October 25 or 26.
For those extending to Jordan, passports must be valid through May 5, 2013.
Cancellations
In the event you must cancel your participation on the tour, contact Linda Freeman,
President, Concierge Services, LLC, at 507-280-9066. Cancellations are subject to per-
person penalties and will depend on when the cancellation is made. Refunds are based
on payments and deposits that Concierge Services, LLC, has made on your behalf from
the time of your deposit and until the time of cancellation. Concierge Services, LLC,
and Linda S. Freeman, strongly recommend trip cancellation and interruption insurance
coverage; all persons placing deposits for pre- and post-scientific education programs
will be given trip cancellation insurance information.
Trip Cancellation Insurance
Trip cancellation insurance is strongly recommended. Trip cancellation information
will be sent to all individuals placing trip deposits at the time the deposit is received.
Cancellation insurance covers your financial investment in the trip (minus the premium
paid) and is strongly recommended for all travelers. Trip cancellation insurance will not
cover ‘changing your mind’ and is generally for immediate family and personal traveler
medical concerns.

Concierge Services, LLC, acts only as an agent for the suppliers and contractors and
group operators providing air and other transportation, hotel, sightseeing, restaurant and
other services, and assumes no responsibility for (no matter how caused) personal injury,
personal or property damage or loss in connection with any services from independent
suppliers that are not subject to the control of Concierge Services, LLC, and Linda S.
Freeman. Concierge Services, LLC, and Linda S. Freeman, can therefore not be held liable
for any accident or personal injury or property damage or personal loss that may occur
in connection with any service due to (1) Any act of omission by such a supplier, or (2)
Any defects in or failure of any aircraft, vessel, automotive vehicle, or other means or
transportation that is not under our control, or (3) Any circumstances arising as a
result of military or political action, weather, acts of God, or any other circumstances over
which Concierge Services, LLC, and Linda S. Freeman, have no control.
Concierge Services, LLC, reserves the right to alter, change or omit any part of the
itinerary or any reservation, conveyance, or routing at any time and without notice,
when it is deemed necessary or desirable by Concierge Services, LLC, and Linda S.
Freeman, and is deemed to be in the best interests of the participants. Concierge Services,
LLC, reserves the right to pass on to tour members any losses caused by delays or
events beyond our control including extreme (more than 5%) cost changes. Concierge
Services, LLC, also reserves the right to decline to accept or retain any person(s) as a
tour member if the circumstances require. Participants are responsible for understanding
there are walking tours included requiring physical stamina and for which alternative
arrangements will not be possible. Good health and physical condition of participants
is assumed by Concierge Services, LLC, at the time of deposit. Participants must inform
Concierge Services, LLC, of any health or incapacity or accommodation issues before
traveling to Israel. Trip cancellation insurance is highly recommended.
