check void if not used within one (1) year from the date rebate

CLAIMS MUST BE POSTMARKED no later than Februar y 1, 2006. This certificate

must be complete and accompany your request. Valid on appliance purchases made

from July 28, 2005 – January 1, 2006.

If delivery has not been made by February 1, 2006: You may submit your claim upon

delivery of your appliance until July 1, 2006 provided the receipt was originally

generated and dated within the program period. Your rebate check will be mailed

after receipt of a valid serial number. ALL CLAIMS SUBJECT TO AUDIT – Please allow

8 weeks for delivery.
