Page 1 BEFORE USING THE COMMANDER FLEX®, READ THE ENTIRE USER MANUAL. The Commander FLEX System has been designed, manufactured and tested according to the highest quality standards. It is being provided to you by your health care team. You are required to return the Commander FLEX, and all peripherals upon request. All accompanying documents including this manual and inserts shall be regarded as part of the Commander FLEX System. The “crossed-out wheelie bin” symbol on the device indicates it must not be disposed of with your normal household waste. Instead, return the products to Cardiocom for the recycling of waste electrical and electronic equipment. Page 2 To ensure the correct and safe use of the Commander FLEX® device (including all of its parts and peripherals), the following safety measures should be followed, along with the other warnings and cautions listed in this manual and on the device. Page 3 DANGER: Do not wrap the hose of the blood pressure cuff, the device power supply cord, or any other portions of the Commander Flex, around the neck. Doing so could result in strangulation or other injury. Do not use the device power supply if it is damaged, or if it plugs into the electrical outlet loosely, or is otherwise improperly connected to an electrical outlet. Doing so may result in electric shock, overheating, fire, or short-circuiting. When using the device power supply, make sure it is properly and fully inserted into the electrical outlet. Failure to do so could result in electric shock, overheating, or short-circuiting. Do not overload power outlets. Plug the device into the appropriate voltage outlet only. Use power supply only with AC 120 volt electrical outlet. Use with electrical outlets at other voltages may result in the risk of fire, electrical shock, or burns. WARNING: Read all of the information in this manual before operating the Commander Flex. Operate the Commander Flex as intended and do not use the device for any other purpose. The Commander Flex is intended for use by an adult. Children should not use the device or be left unattended with the Commander Flex. Do not use the device if you are unable to operate it by yourself. The device is not to be used by persons who cannot express their intentions. Do not insert foreign objects into any openings of the Commander Flex or attempt to modify the device. Do not use the blood pressure cuff where either an intravascular access or therapy, or an arterial (A-V) shunt, is present, as this may cause interference to blood flow and result in an injury.
Page 4 Do not use the blood pressure cuff or any other part of the device on part of your body with an existing injury or wound. Such use may result in further injury. Do not use the blood pressure cuff on an arm on the side of a mastectomy. Doing so may result in further injury. Do not use the blood pressure cuff on an arm while monitoring equipment is already in use, as this can temporarily cause loss of function of the monitoring equipment. If the product does not operate properly, exhibits an error or malfunctions in any way, stop use and unplug the device from electrical power supply immediately, and request inspection and service. Failure to do so could result in the risk of an accident, electric shock, or fire. If your device needs servicing, please call Customer Care at the phone numbers identified on the front page. The Commander Flex is designed to be maintained and serviced (with the exception of cleaning) only by qualified personnel. Do not disassemble or attempt to repair the device. The device is for indoor use only. Do not operate the Commander Flex in extreme temperature or humidity conditions. If the device is used close to televisions, microwave ovens, cellular telephones, X-ray or other electrical devices, Commander Flex measurements or output may be distorted, or device operational failure may occur. Unplug the device power supply from the electrical outlet before cleaning. Do not plug or unplug the device power cord into the electrical outlet with wet hands. Doing so may result in electrical shock or fire. Do not expose the Commander Flex to water or other liquids. Doing so may cause electric shock, short-circuiting, and other damage to the product. Use power supply only with electrical outlet of voltages marked on name plate. Do not use a voltage converter. Doing so may cause fire or electric shock.
Page 5 Do not allow any material to drape over the device power supply while in use. Doing so could result in the risk of fire. Do not modify or attempt to repair the Commander Flex or peripherals. Doing so could result in the risk of fire, electrical shock, or injury. Do not scratch, damage, modify, bend, pull, or twist the power supply cord or the other parts of the Commander Flex. Do not place heavy objects on top of the device power supply cord or pinch it with objects or tools. Do not wrap the cord around the device or the body of the power supply. Doing any of these things may result in the risk of fire or electrical shock. Do not step or walk on the device power supply cord or any cables or peripherals. Doing so may cause an accident or injury. Do not submerge the Commander Flex in water or get it wet (other than the moist cloth used while cleaning.) Blood flow interference and harmful injury can be caused by continuous cuff pressure due to connection tubing kinking. Too frequent of blood pressure measurements can cause injury due to blood flow interference. The Scale Wireless Edition™ is intended for use by an adult. Children should not use the device or be left unattended with the Scale WE. The Scale Wireless Edition™ is designed to be maintained and serviced (with the exception of cleaning) only by qualified personnel. Do not disassemble or attempt to repair the device. The device is for indoor use only. Do not operate the Scale Wireless Edition™ in extreme temperature or humidity conditions. Do not expose the Scale Wireless Edition™ to water or other liquids. Doing so may cause short-circuiting, and other damage to the product. Do not modify or attempt to repair the Scale Wireless Edition™. Doing so could result in the risk of injury.
Page 6 Do not attempt to repair or open batteries. Remove the batteries from the device when there is a long delay between uses. Batteries that are not used for a long time have the potential to leak and cause an injury. Do not mix old and new batteries, batteries of different types, or batteries from different manufacturers. If battery fluid should get in your eyes, immediately flush your eyes with water and seek medical attention. Batteries are choking hazards for children under 3 years old. Do not allow children to play with the device.
Page 7 CAUTION: Cleaning must not be performed while equipment is in use or connected to the power supply. Avoid tightly folding the blood pressure cuff or storing the hose tightly twisted for long periods, as such actions may shorten the life of the Commander Flex components. If the Commander Flex becomes dirty, clean with a soft cloth that is slightly moistened with warm water. Do not use alcohol, paint thinner, or anything other than warm water. Use care in handling the Device. Do not drop the Device or any parts or peripherals. Protect the Device from impact or sudden shocks. Store the Commander Flex in a clean, safe location. Do not store it in direct sunlight, high humidity, freezing temperatures or other extreme temperatures or dust. The effectiveness of this sphygmomanometer has not been established in pregnant (including pre-eclamptic) patients. Make sure to use the Scale Wireless Edition™ on a stable surface. Do not use the scale on carpet without the platform. If the Scale Wireless Edition™ becomes dirty, clean with a soft cloth that is slightly moistened with warm water. CaviCide® wipes can be used on the external surface if available. Do not use alcohol, paint thinner, or anything other than warm water or CaviCide® wipes. Store the Scale Wireless Edition™ in a clean, safe location. Do not store it in direct sunlight, high humidity, freezing temperatures or other extreme temperatures or dust. Page 9 A Welcome to the Commander FLEX System What is the Cardiocom Commander FLEX? Commander FLEX is a home monitoring tool. It sends important health information to your health care team. This allows your doctor and other health care team members to take action before your symptoms get worse. Your health care team can make changes to your care plan to help you stay healthy. How do you participate? Your health care team has provided the Commander FLEX for you to use at home. You should use the Commander FLEX each day. You may use the Commander FLEX to measure your weight, peak flow, oxygen saturation, blood pressure, and/or blood glucose levels. You should do this as instructed by your health care team. This data will be sent to your health care team each day. The Commander FLEX uses its own cellular connection or toll free numbers to send the data so there will be no charge to you. What are the benefits of using Commander FLEX?  Identifies symptoms early and allows your health care team to take action  Keeps you “in touch” with your doctor and his or her team  Teaches you about your health condition Keep this Manual near your Commander FLEX for a quick reference.

You can use your home phone line or the device can use the built-in cellular modem.  If you have a home phone line, turn to Section E for Using Commander FLEX with a Telephone Line.  If you do NOT have a home phone line, turn to Section D for Cellular Connection Setup. Page 17 Commander FLEX Activation When the Commander FLEX is plugged in, it will turn on and start the setup process. This step will take a few minutes to complete. If the Commander FLEX was not able to complete the setup process on its own, the device will give you more instructions. Follow these instructions to complete the setup process. Signal Strength and Cellular Connection If the Commander FLEX has low signal strength, a signal strength meter will be shown on the screen before it tries to send your data. This meter shows the strength of the cellular connection. The more bars you see on the meter, the stronger the signal. Here are a few tips to help you get more bars and improve your signal strength: 1. Place the Commander FLEX near an outside wall or window. 2. Do NOT place Commander FLEX next to your TV or other electrical equipment. 3. Do NOT place Commander FLEX near large metal objects, such as your refrigerator. Now you can turn to Section F for instructions on using the Commander FLEX.
Page 18 E Using Commander FLEX with a Telephone Line Note: You should use this section to setup Commander FLEX if you DO have a home phone line. Where to Set Up Commander FLEX When Using a Telephone Line 1. Place near a working telephone wall jack. 2. Place near an electrical outlet. NOTE: You can check to see if the wall jack is working by connecting a phone to the wall jack. You should hear a dial tone on the phone if the wall jack is working properly. Page 19 How to Connect Commander FLEX When Using a Telephone Line Be sure to connect the parts in this order: 1. If there IS a telephone plugged into the wall jack, unplug the phone from the wall jack and go to Step 3. If there is no phone plugged into the wall jack, proceed to Step 2. 2. If there is NOT a telephone plugged into the wall jack, plug one end of the telephone cord you were sent into the wall jack. Plug the other end into the port on the back of the Commander FLEX labeled “PHONE LINE”. Then, go to Step 6. Wall Jack Wall Jack PHONE LINE Port
Page 20 3. Insert a Telephone Jack Splitter into the wall jack. 4. Plug the telephone into one side of the Telephone jack splitter. 5. Using the telephone cord you were sent, plug one end into the Telephone Jack Splitter. Plug the other end into the port on the back of the Commander FLEX labeled “PHONE LINE.” Your telephone will continue to function as normal. Page 24 F Using Commander FLEX Using Commander FLEX is Easy! Each day you will simply take a Health Check. This may include:  Measuring your vital signs, like weight, blood pressure, oxygen saturation, peak flow, temperature or blood glucose levels.  Answering a series of questions about your health status. That’s it! The Commander FLEX automatically sends your Health Check data to your health care team. The transmission process usually takes less than one minute to complete. More About the Health Check Your Health Check gathers vital sign data and asks a series of health related questions. This health information is then sent to your health care team. The Health Check may also contain messages to teach you about your health condition.
Page 25 How to Start Your Health Check 1. Each day you will press the YES or NO button on the Commander FLEX to turn it on. The following messages will be spoken and appear in the display: “Good Morning” “Are you ready to start your Health Check?” 2. Press the YES button to begin your Health Check. 3. If you have any other peripheral devices attached to Commander FLEX, you will be instructed to take your measurements. Follow the on screen instructions and refer to the device instructions found in this User Manual. 4. The Commander FLEX may ask you a series of questions about your current symptoms. Page 27 G Main Menu The Main Menu can be used to view past vital sign readings and change device settings. Accessing the Main Menu: Press the “i” button during the “Are you ready to start your Health Check?” question to enter the Main Menu. Use the selection buttons on the left to go through the menus. MAIN MENU 1. Review Readings View past vital sign readings. Readings can be viewed in a graph or in a list. 2. Device Setup This section is used to view and change the device settings. DEVICE SETUP MENU 1. User Setup This section allows you to change the volume, language, and home screen. 2. Connection Setup This section allows you to change how the Commander FLEX sends your data. You do not need to go into this menu if your device is working correctly. 3. Advanced Setup This section should only be used when instructed by your health care provider or technical support. Page 28 CONNECTION SETUP MENU The Connection Setup Menu options should only be used when instructed by someone setting up your Commander Flex or by Client Care Services. 1. Connection Method You can change between the Phone Line and Cellular connection types in this menu. 2. Cellular Signal Strength You can view the strength of the cellular signal. This can help you find the best place to set up the device. The more solid bars you see, the better the signal. 3. Phone Line Setup This menu is used to add a number to get an outside line or change the dialing type. PHONE LINE SETUP MENU 1. Phone Number setup You can edit the number the device calls to send data. You can add a number for an outside line, like a 9, or add or remove the area code and 1 for long distance. 2. Tone/Pulse Dial You can change the dialing type between Pulse and Tone dialing. Most new phone lines use Tone Dialing.
Page 29 ADVANCED SETUP MENU The Advanced Setup Menu options should only be used when instructed by someone setting up your Commander Flex or by Client Care Services. 1. Reactivation Resets the device and deletes all data. 2. Diagnostics Used by technical support. 3. Peripherals Allows for changing settings to Blood Pressure or configuring the connection of the Wireless Scale. PERIPHERALS MENU 1. Blood Pressure Used to turn the blood pressure ON or OFF and to set the first pressure level. 2. Wireless Scale Used to link (connect) or unlink (disconnect) the SC200 Scale WE (Wireless Edition). Please note: The menu options may be different as they are dependent upon the firmware version.
Page 30 H SC100 Scale PE (Professional Edition) & SC250 Scale SE (Slim Edition) Unpacking the Professional or Slim Scale 1. Remove the Scale you received from its box. 2. Check for any damage incurred during shipping. If you note any damage, call your care provider as soon as possible. 3. Please save the original box and shipping material for when you return the Scale. 4. If you are going to use the Professional or Slim Scale on carpet, unpack the Carpet Cups or Carpet Platform as shown below. 5. Keep the scale cover on at all times when using the device.
Page 31 Setting Up the Scale PE and Scale SE on a Hard Surface 1. Place the Scale on a hard surface away from high-traffic areas. 2. The Carpet Cups or Carpet Platform should NOT be used when using the Scale on a hard surface. CAUTION: Place the scale on a surface away from traffic areas to prevent a tripping hazard. Setting Up the Scale PE on Carpet 1. Attach the Carpet Cups a. Remove the adhesive cover off of the Carpet Cup b. Press the sticky side of the Carpet Cup firmly onto one of the rubber feet on the bottom of the Scale PE c. Attach the rest of the Carpet Cups 2. Be sure to carefully check the Scale PE’s stability. CAUTION: Carpet Cups should not be used on hard flooring. If moving the scale from carpet to hard flooring, remove the carpet cups from the scale.
Page 32 Setting Up the Scale SE on Carpet 1. Set the Carpet Platform on the carpeted floor. 2. Place the Slim Scale on top of the Carpet Platform according to the directions on the Carpet Platform label. a. Slim Scale feet should be placed in the Carpet Platform foot holders. b. Make sure the cord is not caught between the scale and platform. CAUTION: The Carpet Platform should not be used on hard flooring. Connecting the Wired Scales to Commander FLEX 1. Insert the YELLOW end of the wired Scale cable into the YELLOW port labeled “SCALE” on the back of Commander FLEX. 2. Your Scale connection should look like this: Foot Holders Feet OR
Page 33 Using the Scales 1. You will weigh yourself during the daily Health Check. 2. Commander FLEX will ask you to “Please step on the scale”. Keep your hands at your sides while weighing. Do not hold on to anything nearby or your weight reading may be inaccurate. 3. If you are not on the scale within twenty seconds after the Commander asks you to step on the scale, please wait for the next instruction from the Commander FLEX. CAUTION: Failure to step on the scale correctly may cause you to lose your balance. If you feel unstable on the scale, do not continue use of the scale as this may result in a fall or injury. 4. Commander FLEX may then display the following messages: a. Your weight is ____ lbs.  This is your current weight to one-tenth of a pound. b. Your weight as compared to your last recorded weight.  An example would be: Good Job! You have lost 1.5 lbs. 5. Weigh yourself without clothes or with the same amount of clothes every day. This will help keep your weight measurements consistent. 6. Weigh yourself in the morning after going to the bathroom and before eating or drinking anything. 7. You do not need to remain on the scale to answer the Health Check questions or to measure other vital signs. 8. The Commander FLEX will automatically transmit your weight to your health care team.
Page 34 I SC200 Scale WE (Wireless Edition) The Scale WE is a cable-free (wireless) body weight scale that you can use in your home with or without a Commander Flex. The Scale WE does not come with a cable to connect to the Commander Flex. Please refer to the SC200 Scale WE Quick Start Guide that came in the Scale WE package. Your Scale Wireless Edition™ kit includes: Scale Wireless Edition (Scale WE) Carpet Platform Six (6) AA alkaline batteries

Page 37 Setting Up the Wireless Scale on a Hard Surface 1. Place the Wireless Scale on a hard surface away from high-traffic areas. 2. The Carpet Platform should NOT be used when using the Wireless Scale on a hard surface. Setting Up the Wireless Scale on Carpet 1. Set the Carpet Platform on the carpeted floor. 2. Place the Wireless Scale on top of the Carpet Platform according to the directions on the Carpet Platform label. a. Wireless Scale feet should be placed in the Carpet Platform foot holders. Page 39 Scale WE (Wireless Edition™) Display The Scale Wireless Edition™ display turns on and lights up when at least 50 lbs (or 23 kg) is on the scale. After a measurement, the scale displays the reading and instructs the person to step off. The scale display and backlight will automatically shut off. The weight measurement displays the unit (to one decimal place) and the measurement type (“lbs” or “kg”). The battery level appears on the lower left area in the display when the scale is in use. The transmitting symbol displays when a successful reading has been taken and it is transferring it to the Commander Flex. If the scale is linked or connected to the Commander Flex, the transmitting symbol will appear in the upper left of the display. If the scale is not linked or not connected to the Commander Flex, the transmit fail symbol displays. The Commander Flex will not receive the weight measurement, nor send it to your health care team. If the scale cannot take a measurement due to the person being unstable, an animated stick figure displays in the upper right corner. If the scale cannot take a measurement, it will say “No weight data collected. Please step off the scale.”
Page 40 Linking the Scale WE to the Commander FLEX To be able to send your weight readings to your health care team, follow the steps below to register the wireless scale: 1. Press any button to wake up the Commander Flex. It displays: “Are you ready to start your Health Check?” 2. On the Commander FLEX, select the “Info Button.” It is located in the upper right hand corner with a blue “i”. Selecting the “Info Button” will display the Main Menu. 3. Select Device Setup from Main Menu. 4. Select Advanced Setup from the menu. 5. Select Peripherals from the Advanced Setup menu. 6. Select Wireless Scale from the Peripherals menu. 7. Select Connect to Scale from the Wireless Scale menu. 8. The Commander Flex will prompt you to connect the Wireless Scale. 9. Turn Scale WE over so the bottom is facing you. You will notice the buttons on the back of Scale WE. Please find and press the button with the wireless symbol. If the scale sound is on, you will hear the wireless scale say “Scale Connected”. 10. If a weight is taken before a health check, the Commander FLEX will skip the “Take your Weight” section of the health check. 4. This weight will be used in place of the “Take your Weight” section of the health check. 5. If you have not taken a weight and start your health check, the Commander FLEX will ask you if you would like to take a weight. Page 45 Changing the Scale WE™Batteries The Scale will display a battery level at the time of use. Note that when the batteries are completely depleted, the battery level will not be displayed. Full Low Before the Battery Level is low, replace the batteries in the battery compartment. The battery compartment is accessed by turning over the scale and opening the battery door. 1. Turn the scale upside down. 2. Open the battery door by pushing down on the battery case tab and pulling out. 3. Take out all of the batteries. All batteries are to be replaced at the same time. 4. Replace with six (6) (AA) new alkaline batteries, taking care to insert the new batteries in the correct orientation (+) and (-) as shown inside the battery compartment.

If on carpeting or an unstable surface, please make sure the Carpet Platform is between the carpet and the scale. Check to make sure the feet of the scale are nestled into the footholds of the Carpet Platform. b) Ensure that minimally there is fifty (50) pounds distributed on the scale surface, not to exceed four hundred (400) pounds. c) Replace the scale batteries, if necessary. d) Check to make sure the scale feet are free from debris or foreign materials (such as clothing or towels). For Accurate Blood Pressure Readings 1. Take your Blood Pressure while you are relaxed. If necessary, relax for a few minutes before starting the Health Check. 2. You should be in a seated position, with your legs uncrossed and your feet flat on the floor. 3. Remain still and do not talk during the measurement process. Excessive movement can make it difficult for Commander FLEX to take your Blood Pressure. 4. Eating, drinking or smoking can change your Blood Pressure. Try to avoid these activities for 30 minutes before measuring your Blood Pressure. 5. Make sure to rest your arm between readings. 6. Blood Pressure varies in all people depending on activity, stress level, food and drink intake and time of day. 7. The Blood Pressure Cuff size can affect your readings. If your arm is bigger or smaller than the range printed on the Cuff, ask your care provider for a new Cuff. 8. Do not allow other people to use Commander FLEX to measure their Blood Pressure.
Page 53 About the Blood Pressure Measurement Device Blood Pressure measurements determined with this device are equivalent to those obtained by a trained observer using the stethoscope auscultatory method, within the limits prescribed by the American National Standard, manual, electronic, or automated sphygmomanometers. The Blood Pressure Cuff will get tight around your arm in order to measure your blood pressure. The device is designed to automatically deflate. WARNING: IF THE PRESSURE IS TOO PAINFUL OR THE CUFF IS TOO TIGHT, PRESS ANY BUTTON ON THE DEVICE, REMOVE THE CUFF FROM YOUR ARM, OR PULL THE CUFF HOSE OUT OF THE COMMANDER FLEX. It is helpful to rest your arm and hand on a hard surface while obtaining a reading. 2. The Commander FLEX will prompt you to “Press YES to check your oxygen saturation”. Press the YES button on the Commander FLEX when your finger is inside the Finger Clip. 3. The Commander FLEX will indicate it is “Checking your Oxygen Saturation”.
