The model “Saber" Bluetooth Communication Module Is installed in our company’s own Medical
Electrical (ME) Equipment as original equlpmentt We do not supply a manual or advertising
literature with the Saber module but we do notify our Documentation Department of the required
statements that must be placed in the ME Equipment Instructions For Use (IFU), meeting the
requirements of the Federal Communications Commission CFR 47, Part 15, Paragraph
15.19(a)(3) and Industry Canada RSS-GEN, Paragraph 7.1.5:
A representative copy of thls notification memo is attached.

RE: Radio equipment compliance statements
The model “Saber" Bluetooth Communication Module is installed in our ME Equipment as original
equipment. We do not supply a manual with the Saber module but we request that the following
required statements he plaoed in the ME Equipment manual to meet the requirements of the
Federal Communications Commission CFR 47, Part 15, and Industry Canada RSS-GEN.
COMPLIANCE STATEMENT
“This device complies with Part 15 of the FCC rules. Operation is subject to the following
two conditions: 1) This device may not cause harmful interference, and, 2) This device
must accept any interference received including Interference that may cause undesired
operation."
MODIFICATION CAUTION STATEMENT:
“CAUTION TO USER: Changes or modifications not expressly approved by the party
responsible for compliance could void the usefs authority to operate the equipment."
