1. Themaincircuitboardcarryingtheaboveidentifiedlimitedmodulartransmitterhasbeentestedand

approvedwheninstalledandforoperationwithinPhilipsDreamStationandmaynotbeinstalledand

operatedinotherhostdeviceswithoutpriorre‐assessmentacc.toFCCrules;

2. OnlytrainedassemblersatthePhilipsRespironicsfactoryorauthorizedservicecentersarepermittedto

installthemodule,accordingtoqualifiedPhilipsRespironicsfactoryassemblyinstructions;

3. Themodulemustnotbeeasilyremovable,installable,replaceablebynormalend‐users;hostproduct

(DreamStation)instructionsmaynotcontaininstructionstoend‐usersabouthowtoremove,installor

replacetheboard;

4. Theidentifiedmodulartransmitterhasbeentestedandapprovedwheninstalledandforoperationwithin

PhilipsDreamStationandalsowhentheDreamStationisoperatedtogetherwitheithertheCellular

Accessory(containingCDMAmodulewithFCCIDQIPPCS3)orwiththeWiFiAccessory(containingWiFi

modulewithFCCIDTLZ‐CU288);

Co‐locationwithanyothertransmittersisnotpermittedwithoutpriorassessmentacc.toFCCrules;

5. Aminimumdistanceof20cmmustbekeptbetweenthetransmittersectionsoftheWiFiortheCellular
