The product is intended for general wellness tracking for ages 12 years and older. PRECAUTIONS: 1. Any form of modification to this device is forbidden. 2. Do not use this device together with MRI or CT equipment. 3. Physicians should diagnose based on clinical references and symptoms, and use this device as a subsidiary tool. 4. Do not use this device if it cannot stay in contact with the skin and do not use on wounded or irritated skin. 5. This device is non-sterile. 6. Do not submerge the Vital Scout patch in water. 7. Do not excessively bend or twist the Vital Scout patch. 8. In case of skin discomfort, remove the Vital Scout patch immediately. 9. For additional safety, keep the product out of the user’s mouth. 10. Device setup shall be performed by adults. Please call customer service at (408) 663-6784 if any issues occur during device setup. 4 INSTRUCTIONS FOR USE 4. APPLY ADHESIVE Remove tab “one” of the adhesive and attach it to the bottom of the Vital Scout patch. Press firmly to ensure the adhesive is adhering to the patch. 5. APPLY PATCH Remove tab “two” of the adhesive and apply the patch horizontally to the chest, above the heart. You may also apply the patch vertically in the middle of the chest. Remove tab “three” from the adhesive to finish the application. 6. SET YOUR GOALS Once the patch is paired and attached to the body, you can set daily goals you would like to achieve. These goals will be used to help motivate you to a healthier lifestyle. Keep in mind, these goals can be adjusted later. DOWNLOAD THE APP Download the free Vital Scout app by searching Vital Scout in the Apple App Store or Google Play Store. You must download the app in order to use the product. 2. SIGN UP Create an account and enter your profile data so the app can accurately monitor your health. 3. PAIR THE PATCH Make sure Bluetooth is enabled in your phone’s settings. Place the patch near the phone and start scanning. Select the serial number from the list that matches the serial number that is on the back of your Vital Scout patch. 7 FEATURES DASHBOARD PROFILE The dashboard view allows you to see an overall picture of your current health. The categories represented here are Stress, Activity, Recovery, and Sleep. You can see more details of each category by clicking on the associated tab. In the Profile tab, you can edit your profile information and daily goals. MY DEVICE SETTINGS My Device allows you to check the battery life and connection status of your Vital Scout patch. You can also pair with your patch here. In the Settings tab, you can edit your account information, sign out, review our company policies, get help, and more. NOTES The Notes tab allows you to create journal entries to record recent activity, nutritional habits, symptoms, and more. 5 THE WARNING INFORMATION FOR FCC This device complies with Part 15 of the FCC Rules.
