On Lok Lifeways Opens 5,200 Sq. Ft. Clinic to Create Second PACE Center in San Jose
New $3.3 Million Facility to Support Growing Need for Senior Healthcare Services in the South Bay; Ribbon-cutting Ceremony and Guided Tours Scheduled for Thurs., Jan. 25
SAN JOSE, Calif. (Jan. 17, 2018) According to Age Friendly Silicon Valley, the population of seniors ages 65 to 84 will grow by 175,000 in Santa Clara County by 2030. To help address the growing need for more senior healthcare services in Silicon Valley, Bay Area-based nonprofit, On Lok, is opening a second PACE Center in San Jose. As the pioneering organization that envisioned and created a model of care that is nationally known as Program of All-inclusive Care for the Elderly (or PACE), On Lok will now have capacity to serve more than 350 nursing home-eligible Santa Clara County senior residents across the two sites. A public ribbon-cutting ceremony and guided tours are scheduled for Thursday afternoon, Jan. 25 at 130 North Jackson Avenue in San Jose.
"In line with On Lok's core mission of caring for seniors and helping them stay in their homes for as long as possible, we knew it was important to increase the number of senior services available in Santa Clara County and support its fast-growing, over-65 population," said On Lok CEO, Grace Li. "On Lok is proud to continue investing in the communities it serves by expanding and bringing more services in areas where there is need."
The East San Jose PACE Center, which complements San Jose's existing PACE Center and five others across the Bay Area, is part of the On Lok Lifeways program, which provides seniors with a wide range of all-inclusive care services. Among them: primary medical care, physical therapy, dentistry, podiatry, optometry, nursing services, mental health counseling, prescription drugs, nutrition services and transportation.
Designed by Alameda, Calif. based MBH Architects and built by Swinerton Builders, the $3.3 million, 5,200 sq. ft. clinic features exam rooms, a two-bed treatment room with observation window, clinic reception area, meeting rooms for specialists, central nursing station, phlebotomy room, doctor's office, nurse's office and utility spaces. Energy efficient lighting controls have been installed throughout the building. On Lok is also adding staff to fill multiple positions, from physicians, registered nurses, certified nurse assistants and social workers to physical therapists, registered dieticians, rehabilitation aids, activities staff and drivers.

About Program of All-Inclusive Care for the Elderly (PACE) The PACE model of care was developed by On Lok through a series of successful demonstrations in the 1980s. On Lok led a national demonstration effort testing the models across the country which culminated with the passage of The Balanced Budget Act of 1997, making PACE a permanent provider type under Medicare and a state option under Medicaid. Today, PACE has grown nationally to include 122 programs in 31 states, and PACE organizations in California serve more than 7,000 participants at 37 PACE Centers and Alternative Care Sites.
