A WARNING identifies situations or actions that may have an
effect on patient or user safety. To ignore a warning could
cause patient or user injury.
A CAUTION identifies special procedures or precautions that
persons must obey to help prevent equipment damage.
IMPORTANT SAFEGUARDS
When using electrical products, especially when children are present,
basic safety precautions should always be followed, including the
following important safeguards.
READ ALL INSTRUCTIONS
BEFORE USING
Training shall be provided by a Hill-Rom qualified trainer or respiratory
therapist before the user uses the product for therapy delivery. The
training may last 60 minutes or until the user is comfortable with the safe
and effective use of the product.
The Monarch™ product is intended for Single Patient Use (SPU) in the
Home Care environment.
Always unplug this product immediately after use or after you charge the
battery.
Remove the rechargeable battery pack from the product if the product is
not going to be used for an extended period of time.

Important Safeguards
WARNING:
To reduce the risk of burns, electrocution, fire, or personal injury, follow
these instructions. Failure to do so could cause personal injury or
equipment damage.
1. Patients that may have difficulty clearing secretions from the upper
airway (such as those with DMD or other advanced neuromuscular or
neurological disorders) may require specialized therapy regiments
involving manually or mechanically assisted coughing or other
techniques in conjunction with Monarch™ Airway Clearance System
therapy. Please consult your physician to determine if additional
therapy is appropriate.
2. It is recommended that a single layer of cotton clothing be worn
beneath the wearable garment.
3. Close supervision throughout the treatment is necessary when this
product is used by patients with physical limitations or impaired
cognitive abilities.
4. Do not store or use the device around pets, pests, or unsupervised
children.
5. We recommend your first use of this product be in a supervised
setting such as at home with a clinical trainer or in a doctor’s office.
6. Use this product only for its intended use as described in this manual.
7. Do not use attachments not recommended by the manufacturer.
8. Do not use unauthorized parts or accessories with this device.
9. Use only Hill-Rom authorized garment vest shells to avoid allergic
skin reactions.
10. If this product has a damaged Power Cord or plug, or the product
does not operate correctly, do not use it. For examination and repair,
contact Hill-Rom.
11. Store and keep the Power Cord out of reach from toddlers and young
children to prevent strangulation risks.
12. Keep the unit, battery, AC/DC power adapter, and Power Cord away
from heated surfaces.
13. Do not use the device near to any heat source such as a fireplace or
radiant heater.
14. Do not use the device in a dusty environment.
15. Do not use the device in a heavy moisture environment.
16. When you remove the product from storage conditions, let the
Monarch™ Airway Clearance System return to its recommended
operating temperature range before you use the product.
17. Read and follow all instructions for setup, use, and cleaning of
Monarch™ Airway Clearance System. Failure to do so could cause
patient injury or equipment damage.
18. Machine wash and dry the vest shell monthly or when visibly soiled.

19. If conditions exist that prohibit use of the Monarch™ Airway Clearance
System, do not use the unit. Death or serious injury could occur.
20. Federal USA law restricts this device to sale by or on the order of a
physician.
21. Only authorized persons should service Monarch™ Airway Clearance
System.

22. The potential for electrical shock exists with electrical equipment.
Failure to follow protocols may cause death or serious personal injury.
23. Make sure the position of the Monarch™ Airway Clearance System is
such that you can quickly, without obstruction, disconnect the power
cord from the main power supply if necessary.
24. Do not expose the unit to excessive moisture or immerse it in water.
25. If the display on the Monarch™ pendant is faulty, do not use the unit.
26. If the Monarch™ Airway Clearance System is wet or damp, do not plug
it into a power source or use it.
27. Before cleaning, unplug the unit from its power source and remove
the battery.
28. Do not use harsh cleansers, solvents, or detergents.
29. Retain the original packaging material. If shipping is required, the
original packing material provides the best protection for the
Monarch™ Airway Clearance System.
30. Do not eat or drink during a therapy session. To do so could cause a
choking hazard.
31. Discard Monarch™ Airway Clearance System product in accordance
with your local government regulations.
32. This product contains chemicals known to the State of California to
cause cancer and birth defects or other reproductive harm.
33. The maximum temperature of the Monarch™ Airway Clearance
System applied parts will not exceed 109° F (43°C) under all
conditions.
34. Do not allow metallic conductive objects to contact the battery
terminals. Do not short circuit a battery or throw it into a fire; it can
explode and cause severe personal injury.
35. Improper battery use may result in a fire, explosion, or other hazard.
36. Risk of fire and burns: do not open, crush, or heat the battery above
140°F (60°C).

Intended Use
INTENDED USE
The Monarch™ Airway Clearance System is intended to provide Airway
Clearance Therapy and promote bronchial drainage where external
manipulation of the thorax is the physician’s choice of treatment. It is
indicated for patients having difficulty with secretion clearance, or the
presence of atelectasis caused by mucus plugging.
This manual includes instructions for setup, use, and cleaning of the
Monarch™ Airway Clearance System, Model 1000. It is to be used as a
reference guide. Please review all the sections carefully before you use the
system.
INTENDED PATIENT POPULATION
The Monarch™ Airway Clearance System, Model 1000 is intended for
adolescent to adult population (15 years and older) or as prescribed by a
physician.
INTENDED USERS
The users of the system are patients in the home care environments. They
are expected to operate the device to self-administer therapy treatment,
handle the setup, cleaning, and storage of the device.
Users should have the ability to read and understand western Arabic
numerals and the native language used in the manual. We recommend
that the users of the device have an 8th grade education level or
comparable. We recommend that patients who do not meet this
education level get assistance from Hill-Rom qualified trainers before
using this device for therapy.
The patient should also have the ability to start or stop the therapy
physically either via the pendant, mobile app buttons, or pause/resume
control on the garment.

Contraindications
CONTRAINDICATIONS
WARNING:
If any patient conditions exist that could cause the use of Monarch™
Airway Clearance System to present a risk to the patient, do not use the
unit except as directed by a physician. Death or serious injury could
occur.
Monarch™ Airway Clearance System is contraindicated if these
conditions are present:
•
Head and/or neck injury that has not yet been stabilized
•
Active hemorrhage with hemodynamic instability
RELATIVE CONTRAINDICATIONS
According to the American Association for Respiratory Care (AARC)
Guidelines for Postural Drainage Therapy, the decision to use the unit for
Airway Clearance Therapy requires careful consideration and assessment
of the individual patient’s case if the following conditions exist:
•
Intracranial pressure (ICP) greater than 20 mm Hg
•
Recent spinal surgery or acute spinal injury
•
Bronchopleural fistula
•
Pulmonary edema associated with congestive heart failure
•
Large pleural effusions or empyema
•
Pulmonary embolism
•
Rib fractures, with or without flail chest
•
Surgical wound or healing tissue or recent skin grafts or flaps on
the thorax
•
Uncontrolled hypertension
•
Distended abdomen
•
Recent esophageal surgery
•
Active or recent gross hemoptysis
•
Uncontrolled airway at risk for aspiration such as tube feeding or
a recent meal
•
Subcutaneous-emphysema
•
Recent epidural spinal infusion or spinal anesthesia
•
Burns, open wounds, and skin infections on the thorax
•
Recent placement of transvenous or subcutaneous pacemaker

Instructions for Use
SETTING UP THE UNIT
WARNING:
To help prevent injury and/or equipment damage, obey these:

Inspect the garment and accessories before each use. In addition,
after each cleaning cycle, visually inspect each component for
any wear, tear, or deformity. If you have any concern about a
component, do not use it, and replace that component before
the next therapy session.
If the garment is wet or damp, do not plug it into a power source
or use it.
Use the supplied battery charger only. Failure to do so may cause
a fire, which may result in personal injury and property damage.
CAUTION:
Do not open, crush, puncture, incinerate, immerse in water, or heat over
140°F (60°C) the battery.
NOTE:
If your physician has prescribed aerosol therapy to be used during the
treatment, set up the appropriate equipment.
Getting Started
Install Battery
CAUTION:
Obey these cautions to help prevent equipment damage:

Use only with the Hill-Rom approved battery (Hill-Rom part
number 194566 (Inventus Power, Inc. model number 194566)).

Fully charge the battery to 100% before first time use.

Charge Battery
CAUTION:
Use only with the Hill-Rom approved AC/DC power adapter (Hill-Rom
part number 195500 (MEGMEET model number MANGO120-24CK-HR))
to prevent equipment damage.
1. Plug the power adapter into the garment.
2. Plug the power adapter into the power cable.
3. Plug the power cord into an applicable power outlet.
CAUTION:
If the battery charge is below 20%, you may not be able to complete
your therapy session.
NOTE:
The Power Cord required for your country may be different than shown.

Attaching the Vest Shell
1.
Lay the garment on a flat surface.
2.
Lay the right and left vest shell panels, colored side up, on a flat
surface.
Align and put one side of the vest shell on to the garment. For the
left vest shell, slide the pendant dock through the slots on the vest
shell.

Snap the three snaps along the side of the vest shell closest to the
zipper.

Attach the hook and loop fastener
straps around the cam buckle to secure
the bottom of the vest shell to the cam

shell.

Repeat for the other side of the garment.

Machine wash and dry the vest shell as needed (see “General

Measure the patient’s mid chest with a flexible tape measure. Place the
end of the tape measure on the patient’s side, below the bust line. Pull the
tape measure taunt around the mid chest as shown in the image on page

SHOULDER ADJUSTMENT

Open the shoulder pad, and remove
the pad from the garment to free the
shoulder straps for adjustment.
Slide the top stretchable band over to
access the adjustable strap.

Separate and adjust the shoulder strap
to fit the patient’s shoulder height. See
“Garment Fitting” on page 20.

Once the strap is set to the correct orange line, firmly press the
shoulder strap together to engage the hook and loop fastener.

Slide the top stretchable band over the adjustable shoulder strap.

Put the cushion of the shoulder pad
under the shoulder straps.

The PODs should rest comfortably on the upper chest and back of the

CONFIRM GARMENT SIZING ADJUSTMENTS
11. Put on the garment one shoulder at a time like you are putting on a
backpack.
12. Fully zip the front zipper closure.
13. Close the cam buckle on both sides.
•
To close—hold and pull the cam buckle in the direction of the
arrow (A) to close the cam buckle (B). See the image below.
•
To open—hold and pull the cam buckle in the opposite
direction of the arrow to open the cam buckle.
NOTE:
The garment should fit snug against the body but not make it difficult
to breathe.
If the garment is either too tight or loose in the torso, adjust the torso
band. If the PODs are not fitting comfortably against the body, adjust
the shoulder straps to either be longer or shorter.
NOTE:
First disengage the cam buckles, and then unzip the garment. Remove
one shoulder at a time, like you are removing a backpack.

Set Up the Mobile App
SET UP THE MOBILE APP
NOTE:
For assistance with the installation of the mobile application (app) to a
device, contact Hill-Rom: in the USA, call 800-426-4224; outside of the
USA, contact your distributor or local Hill-Rom representative, or go to
www.respiratorycare.hill-rom.com.

From your mobile device, go to your device’s app store.
In the Search box, enter “Monarch System” to find the app.

Choose to get or download the Monarch System app, and follow the
guided instruction to load the free app to your mobile device.

Tap the Monarch app icon to open the
app on your mobile device.

When you first open the Monarch app,
you will have two options to choose
from:
a. I have a Monarch
or
b. Take Me to Demo Mode

If you choose to demo the app without connecting to a Monarch
device, tap Take Me to Demo Mode.
NOTE:
In Demo Mode, all screens are in gray to show no connectivity to a
Monarch device.

If you want to connect to your Monarch device, tap I Have a
Monarch.
NOTE:
Make sure that Bluetooth® is enabled on your mobile device before you
select I Have a Monarch.

Enter the serial number of your

number location.

The Monarch app needs to have
specific parameters enabled to
operate successfully with different
mobile devices. You will see one or two
pop-up messages asking for
permissions to be allowed on your
mobile device. Tap Accept on these
messages.

Monarch app will notify you. Tap Turn
on, and follow the on-screen

CAUTION:
The first time you launch the mobile application a Notification screen
will show. Press OK to allow notifications.
NOTE:
Notifications allow the Monarch app to send information to your phone
(via pop-up messages) even when the app is running in the background.

Program the Garment
PROGRAM THE GARMENT
The pendant is used to program the garment and start the therapy.
The pendant dock is on the front of the garment. Place the pendant in the pendant
dock when the pendant is not in use.
TURN GARMENT ON
1.
Slide the pendant up and away from the garment to remove the
pendant from the pendant dock.
2.
Press and hold the Power button for up
to 5 seconds to power the pendant. You
will see the Monarch System startup
screen momentarily. This screen shows
the software version and the hour
meter reading.
NOTE:
The pendant screen will dim after 3 minutes if there are no user inputs
when using the battery. Any button press will return the screen to
normal brightness.
The Monarch™ Airway Clearance System will shut down after 10 minutes of no activity.

Mode of Operation
Description
Manual mode
Manual mode uses one frequency and intensity for a set time of the entire therapy session.
Program modes
Therapy 1 and Therapy 2 are programmed
therapy sessions. Each program is a quick
start therapy session. A single therapy session can have up to eight steps. You can also
set cough pauses (reminders to cough) in
these therapy sessions.
PROGRAMING MANUAL MODE
Manual Mode will run one frequency and one intensity for a set amount
of time. The therapy settings can be changed at any time during
therapy. The treatment continues while the user adjusts different
frequencies, intensities, and duration values. You can program the
Monarch™ Airway Clearance System through the pendant or a mobile
device.

select Manual therapy, and press the Center
button to confirm selection.

Cleaning
GENERAL CLEANING—GARMENT
We recommend to wipe clean the garment with the recommended
cleaning solutions and warm water. Do not use excessive liquid or harsh
cleansers, and do not submerge The Monarch™ Airway Clearance System
in water or any solution.
Dilute the cleaning solution as specified on the manufacturer’s label.
WASHABLE VEST SHELL
To remove difficult spots or stains, use standard laundry cleaner and either
hand wash or machine wash in cold water. Then either line dry or tumble
dry in the dryer with low heat. To loosen heavy, dried-on soil, you may first
need to saturate the spot.
Machine wash and dry the external vest shell monthly or when visibly
soiled.
NOTE:
Only the Vest Shell can be machine washed. The Shoulder Strap Pads,
Backpack Cover, and POD Covers should be wiped clean with an approved
cleaner.
1. Remove the Vest Shell from the Garment.
2. Fasten all the hook and loop fastener straps to each other to limit
snagging.
3. Machine wash, or hand wash, in cold cycle only (86°F (30°C)
maximum). Non-chlorine bleach may be used as necessary.
4. Hang dry or tumble dry on lowest temperature (120°F (49°C)
maximum).

The Monarch™ Airway Clearance System is a HFCWO therapeutic device
that is used to aid mobilization of secretions from the airways and help
improve airway health. This is achieved by advanced microprocessor
control of 8 PODS which oscillate against the chest wall. The PODs provide
a targeted kinetic energy to the lungs which thin mucus and generate
airflow to move the mucus from the small airways to the large airways
where it can be coughed out or suctioned.
No user initiated calibration is required during the service life of the
Monarch™ Airway Clearance System.
WARNING:
Only authorized person should service the Monarch™ Airway Clearance
System. Service by unauthorized persons could cause personal injury or

The expected service life for the Monarch garment is five years from date
of purchase.
The expected service life of the battery is one year from date of purchase.
Hill-Rom will repair or replace a defective unit as part of a warranty
package specified at the time of purchase of the Monarch garment.
NOTE:
For proper disposal, make sure to follow your local, state, or country
electronic and battery regulations to safely discard or recycle the
Monarch™ Airway Clearance System, parts, and batteries.

found via the pendant (see “Device Information” on page 59), or the serial
number can be found on the product identification label. The product

STORAGE AND HANDLING
To store or transport the Monarch™ Airway Clearance System, do as
follows:
1. Place the garment in the carry case.
2. Insert the AC/DC power adapter, power cord, battery, and other
accessories into the compartments above the garment in the carry
case.
3. Close the carry case to secure the garment inside.
SHIPPING THE UNIT
WARNING:
To help prevent injury and/or equipment damage, obey these:

Do not ship the battery with over a 30% charge.
•
Do not store the battery at temperatures above 140°F (60°C),

– The Frequency of treatment sessions depends on your
underlying disease, age, and state of health. Your physician
will prescribe a schedule for your individual needs.

BATTERY ACCESS PANEL REPLACEMENT
1.
Lift the battery access panel.
2.
Hold the battery access
panel close to the hinges,
and slowly pull the access
panel away from the
backpack to remove the
hinges from the backpack.
3.
Hold the new battery access
panel with the logo facing
away from the backpack.
4.
Insert the left hinge of the
access panel on to the
backpack until it clicks, and
then insert the right hinge
until it clicks.
5.
Press on the access panel until it clicks closed.
POD COVER REPLACEMENT
1.
Lay the garment on a flat
surface.
2.
Unzip the garment, and
open it so that you can get
access to the inside of it.
3.
From the inside of the
garment, hold the POD
cover with one hand, and
gently squeeze the cover
while you pull it straight
away from the garment to
remove the cover.
4.
Hold the new POD cover so that the larger recess in the inner cover
aligns with its mate on the internal ring of the POD. The three outer
recesses should align with their respective screws.
5.
Press the POD cover gently into the POD ring and over the POD.
Press the cover fully in position such that all sides of the cover touch
the garment.

SHOULDER PAD REPLACEMENT
1.
Disengage the shoulder
pad’s hook and loop
fastener, and then remove
the pad from the garment.
2.
Put the new pad into
position on the garment,
and firmly press the
shoulder pad together to
engage the hook and loop
fastener.
BACKPACK ELASTIC COVER REPLACEMENT
1.
Lay the garment on a flat surface.
2.
Remove the battery access panel. See “Battery Access Panel
Replacement” on page 98.
3.
Start at the bottom of the backpack, and lift the elastic skirt out from
under the edge of the backpack.
Continue up both sides of the backpack elastic cover to fully remove
it from the backpack.
4.
Align the new cover with the backpack, and then do the removal
procedure in reverse order. Make sure that the stretchable skirt is
secured completely around the edge of the backpack.
