The program combines mySugr’s data-rich mobile app, personalized patient and clinician coaching, diabetes supplies, and population health management services through the Care Innovations’ platform. “This new partnership with a community healthcare association shows the evolution of mySugr, from an app-based solution to a comprehensive program that can support people living with diabetes in underserved areas,” said Frank Westermann, CEO and co-founder of mySugr. “We welcome this opportunity and look forward to making a meaningful difference in health outcomes.” The Montana Primary Care Association (MPCA) is a non-profit association working to increase access to high-quality, affordable primary health care on behalf of Montana’s community clinics. It oversees a vast geographic area that experiences extreme weather conditions. Many patients drive hours to see their doctor and even further to access hospitals, specialists and supplies. “Both Northwest Community Health Center and Bighorn Valley Health Center are in and serve rural counties,” said Cindy Stergar, CEO of the Montana Primary Care Association. “That creates all sorts of logistical challenges, particularly when treating patients with chronic conditions. There is a clear need for a comprehensive digital diabetes solution to better connect these health centers with the community so that they can provide integrated primary care.” The initial collaboration will focus on two health centers; Bighorn Valley and Northwest Community Health Center. Along with integrated digital solutions, patients enrolled in the program will receive supplies – such as blood glucose strips – and targeted diabetes advice. “This program is all about adapting healthcare delivery, so we can reach individuals living with chronic conditions at a time and place that works for them,” said Randy Swanson, CEO of Care Innovations. “This commitment to each patient, over time, can drive better health outcomes at a population level.
