Descriptive Information : Recall population was determined based on Meritor's identification of line-sequenced assemblies produced with TSE-supplied chambers before TSE's heat treat fixture was corrected.

Description of Defect :
Description of the Defect : Brake chamber with mis-aligned park spring may cause fatigue hole in diaphram and internal air leak.

Description of the Safety Risk : Air leak could drain air system pressure, causing brake drag and setting park brakes, increasing the risk of a crash
Description of the Cause : Root cause was determined to be the deformation of the spring after the heattreat process due to the heat-treat fixture allowing springs to touch each other. The perpendicularity of the spring can cause the pressure plate to be misaligned and cause a fatigue hole in the diaphragm. TSE fixed the heat-treat fixture as well as tightened the allowable tolerance and frequency of inspections during heat-treat process
Identification of Any Warning A variety of warnings can occur, including low air warnings, difficulty going that can Occur : into gear due to low air pressure, audible air leaks and inability to build up enough pressure to release park brakes.

Chronology : 4/5/21 - Meritor informed Peterbilt Product Safety & Compliance that their brake chamber supplier had a potential quality issue with 52 brake chambers that were installed on Peterbilt trucks. 4/6/21 - Meritor and Paccar met to discuss details of the issue, containment and suspect population. 4/7/21 - Meritor provided summary of their testing of the suspect brake chambers and more information from Walmart's experience with the suspect brake chambers. 4/12/21 - 4/23/21 - Peterbilt Product Safety and Compliance communicated with Meritor regarding issue and suspect brake chamber list. 5/3/21 - Meritor sent a copy of their 573 defect report filed April 08, 2021. 5/4/21 - PB Safety Committee met and determined a defect relating to motor vehicle safety exists.
Description of Remedy : Description of Remedy Program : Peterbilt dealers will replace suspect brake chambers on trucks in the field free of charge.
